• Accomplished admissions form

• One (1) valid government issued ID of enrollee and guardian

• Photocopy of Birth Certificate

 

Other requirements (can be submitted after enrollment):

• School record (original copy from last school attended)

• Photocopy of Marriage Certificate, if married

• Six (6) updated passport size photos with white background, collar, and name tag

• Accomplished medical laboratory result and medical certificate

• Medical examination (Physical Examination, Chest X-ray, Hepatitis B Screening, Fecalysis, Urinalysis, Typhoid Test)
